Previous school record form
Three passport size photographs and two stamp size photographs of the students
Two Passport Size photographs of the parents/ guardians
Copy of the birth certificate
Original transfer certificate of the previous school.
Original conduct or character certificate
One passport size photograph of each of the approved visitors.
